What makes a dog at risk for heart worm?

How are heartworms transmitted? I mean, if I have an elderly pug that sleeps on the couch 98% of the day.... do I really need to spend $70 on heartworm pills anymore? I already decided to stop vaccinations this year and my vet seemed OK with that.
